Children who...

•  will be 5 years-old on or before September 30, 2012
•  live in the Bowling Green City School District

If you are a resident of another school district, your child MUST register with that school district. This applies even if you have child(ren) already attending through the interdistrict open enrollment process at BGCS. No exceptions.

Parents are required to bring the following documents at Kindergarten Registration:

• Child's birth certificate - original certificate with embossed seal (we will copy and return to you)
• Child's Social Security Card - original card (we will copy and return to you)
• Proof of required immunizations - see requirements
• Proof of residency - current utility bill, rent receipt, phone bill, etc. containing custodial parent/guardian name, address, and current date
• Proof of custody (if applicable) - original court document stating custody guidelines
• Parent Photo ID - driver's license, student ID, etc.

Parents may expedite the process by completing these forms in advance and bringing them to registration:

• Registration Checklist (pdf) - please bring with you
• Registration Form (pdf) - due at registration
• Emergency Medical Form (pdf) - due at registration
• Bus Transportation Form (pdf) - due by the end of May -- should be filled out even if you plan to transport.
• Ohio School Health History Form (pdf) - due by Kindergarten Orientation in August
• Permission to Release Student Information (pdf) - due at registration
• Medication Form (pdf) - for students requiring medication (prescription or non-prescription) during the school day
• Asthma Inhaler Form (pdf) - for students requiring an inhaler during the school day
• Epinephrine Form (pdf) - for students requiring an epinephrine auto-injector during the school day

The days and times are specific to each building. It is important that you come at the time you have been scheduled to hear the presentation by the teacher(s) from the building your child will attend.
